CDA Board of Directors

Each candidate is required to complete this form for the CDA election. This information is intended to provide CDA Members with a standardized biographical profile of each candidate.

The information requested includes:

  • Academic affiliations
  • Clinical affiliations
  • Medical school and year of graduation
  • Residency training and year of completion
  • Professional activities – offices, appointments and honours with CDA
  • Other national, provincial and local dermatology societies
  • Other medical society, governmental and/or professional appointments, awards and honours
  • Other professional/academic activities
Optional Documentation

Each candidate may provide a Personal Statement (maximum 300 words) outlining their thoughts on the future of dermatology and/or the future work of the Canadian Dermatology Association.

Each candidate may also provide one letter of support (maximum 250 words) authored by a voting member of the CDA of the candidate’s choice. This letter of support is meant to provide the candidate with an opportunity to inform the membership of special qualifications and/or helpful information and insights about the candidate that would not have been read in their biographical profile or personal statement.

The above mentioned documents along with the candidate’s information will be placed on the Members’ section of the CDA website.
